Your inbox fills with "collab?" messages and profile links, each with different details. I would rather read the same answers from every creator. One link asks the same questions in the same order, so you can compare applicants side by side.
About their channels. Name, main platform, a profile link and a follower range. This is how you check the account is real and the right size for you.
About their audience. Niche, country and typical views. Views on a normal post tell you more than a follower count does.
About working together. Why they like your brand, content ideas, past deals and what they expect in return. A final box confirms they will tag gifted or paid posts.
Who fills it and who reads it
Creators fill it, or hand it to their own AI. You, or whoever runs your ambassador program, read the CSV. The creator's AI might ask "Where do you post most?" Then "What would you make for us?" Then "Are you hoping for free product or a fee?" It shows all answers and sends only after a yes. See agent-fillable forms.
A tip for the shortlist
Sort the CSV by platform, then by typical views, and reply to a short list first. Browse forms for creators and podcasters and forms for ecommerce stores. A UGC creator application fits brands that buy content, and a podcast guest form fits creators who also have a show.
Questions
Why does the form ask for typical views and not only followers?
Views on a normal post show how many people really watch. Put the typical views column next to the follower range in the CSV and you see both at once.
Should I collect shipping addresses here?
This template leaves them out on purpose. Collect an address only from the creators you pick, and only when you are ready to send a product.
Can I use it as a brand ambassador application form?
Yes. Change the form title and the question about what the creator hopes for so it matches your ambassador program. Everything else works as it is.
