A paper permission slip goes home on Sunday and comes back crumpled on Wednesday, with a phone number nobody can read. By then the first night has started. I would rather send parents one link and get clean rows.
How it works
- A parent gives the young person's name, school and grade.
- They add their own contact details and one more adult for emergencies.
- They share allergies and needs, and say how the young person gets home.
- They tick photo permission and the permission box, then send.
What a parent's AI would ask
"Which grade is your child in?" Then "Who collects them after the meeting?" Then "Are there any allergies we should know about?" It lists every answer and sends only after a yes. Read about agent-fillable forms.
Good to know
Ask only what you will use. This form leaves out dates of birth and medical history on purpose. Allergies and needs are enough. Filter the CSV by the ride home answer before each meeting, so nobody is left waiting.
Find more in forms for nonprofits, forms for school programs and forms for churches. Adult helpers can apply with a volunteer application. A scholarship application helps families who need support with trip costs.
Questions
What if a parent has two children to register?
Each child needs their own response. A parent can send the form twice, or their AI can fill it twice. Group the CSV rows by guardian email to see siblings together.
Why is there no date of birth?
Grade and a guardian contact are enough for most youth programs. Leaving out extra personal data means less to protect. Add a date field if your insurer or an activity needs it.
Can I close sign-up when the group is full?
Yes. Set max_responses to your capacity, or set close_at to the last day for sign-up. Either way the public form stops taking new answers.
