Team building days often miss because nobody asked what the team needs. A trust game does not help a team whose roles are muddy. This questionnaire asks first, so the day, the workshop or the lunch fits the real gap. I think that is the whole trick.
How it works
- People name their team.
- They rate trust, open disagreement, clarity of roles and how much they enjoy working together.
- They pick the quality that is strongest and the topic to work on first.
- They pick activities they would enjoy, and say what they would rather skip.
- One open box takes an idea.
A short chat
"Which team are you in?" Then "How easy is it to ask a teammate for help, from 1 to 5?" Then "What should the team work on first?" The AI shows the answers and asks "Send it?" first. Read about agent-fillable forms.
Good to know
Ratings from a small team are easy to trace, so report the average, not the rows. It works as a team feedback form between away days too. Go wider with the staff survey questionnaire, or check on strain with a stress assessment form. The employee engagement survey covers pay and growth. See forms for HR and recruiting.
Questions
Can I run it before an offsite?
Yes. Send the link a week ahead, then use the activity picks and the first focus choice to plan the day. Close the form before you build the agenda.
What if some people dislike team activities?
The form asks which activities people would enjoy, lets them say what they would rather skip, and includes a no-activity choice. Honour the picks you can.
How do I compare teams?
Ask each team to use the same form and fill in team_name. Then group the CSV by that column and compare the average ratings for trust and clarity.
