Adopters write long emails, short emails or none, and you need the same facts from everyone. I would rather read answers in the same order for every applicant. Then the call can start from the home, not from guessing.
This form fits if
You place dogs, cats or small animals and you review applicants yourself. You want facts about the home, the household and the time an animal would spend alone.
This form is not enough if
You need identity checks, inspection reports, fees or a signed contract. The form does none of those. It is a form, not legal advice. Have a professional check your adoption contract.
What it asks and why
- Animal and contact: which animal, and how to reach the applicant.
- Home, tenure and landlord box: a renter needs permission to keep a pet.
- People, children and current pets: whom the newcomer must meet.
- Hours alone and reason: whether the daily routine suits the animal.
- Vet clinic: a clue about past care you can phone.
- Visit and return boxes: the applicant agrees to a visit or call, and to contact you before rehoming.
What an applicant's AI would ask
"Which animal are you applying for?" Then "Do you own or rent?" Then "How many hours a day would it be alone?" It shows the answers and sends only after a yes. See agent-fillable forms.
Browse forms for pet businesses and rescues and forms for nonprofits and charities. A pet foster application finds temporary homes, and a lost pet report helps with strays you take in.
Questions
Can I use it for dogs and for cats?
Yes. The animal is a text answer, so the same form works as a dog adoption application form or a cat one. Add a question about gardens or windows if it matters to you.
How do I handle many applicants for one animal?
Download the CSV, filter by the animal_applied_for column and read the hours_alone and home answers side by side. Then call the applicants whose homes fit.
Does the form check references or take fees?
No. It collects answers only. You call the vet clinic yourself, and you take any adoption fee the way your rescue already does.
