A quiz gets people to say what they like, and that is worth more than another sign-up. I should be plain about what this form does. It collects the answers. It does not score them, branch, or show a result page.
Who is taking the quiz
First name and email, plus an optional tick for tips. That gives you a list built from people who actually answered something. The quiz funnel ends in your inbox, because you send each person their result.
The questions
Every question is a single choice about a small travel moment. The options always run in the same order. The first fits Planner, the second Explorer, the third Relaxer and the fourth Foodie. Swap in your own types and theme.
Counting up the result
Download the CSV and count which position each person chose most. A spreadsheet formula does it. You can also give the CSV to your own AI and ask for each person's type. Then you email the result yourself. The form sends nothing back.
Who fills it in
Visitors take the quiz. A visitor's AI might ask: "What do you do the night before a trip?" Then "What is your first move in a new city?" Then "What would a friend call you?" It shows the answers and sends them only after a yes. See agent-fillable forms.
Pair it with a prize
Run a giveaway entry form next to the quiz to bring in more answers. Creators can share the quiz if they apply through a UGC form. More ideas are in forms for creators and podcasters and forms for ecommerce and Shopify stores.
Questions
Does the quiz show people their result?
No. The form has no scoring, branching or result page. You count the answers in the CSV and tell each person their type yourself.
How do I count the result types?
Keep the option order the same in every question. In the CSV, count which answer position a person chose most often. That position is their type, and ties are yours to settle.
Can I use my own theme?
Yes. Rewrite the questions and options before you publish. Keep the same number of options in the same order in each question, so counting stays simple.
