New families arrive in August with a pile of paper and one pen. I would rather send a link in spring. Parents fill in the details at home, or hand the form to their own AI. You get the class list as rows.
Who it is for
School offices, small independent schools, co-ops and after-school programmes that enrol children once a year and need clean contact details.
What each question is for
- Name, birth date and grade: to place the child in the right class.
- Previous school and home language: helps teachers plan support early.
- Guardian name, phone and email: one adult you can reach two ways.
- Home address and transport: for bus lists and drop-off plans.
- Allergies or learning needs: things staff must act on from day one.
- Photo permission and confirmation: a clear yes or no from the family.
What a parent's AI would ask
"What is your child's date of birth?" Then "Which grade is your child entering?" Then "Which phone number should the school call?" It shows every answer and sends only after a yes. Read more about agent-fillable forms.
Before you use it
Files cannot be uploaded here, so collect proof of address another way. More ideas are in forms for schools. A student profile helps teachers get to know each child after enrolment, and a summer camp registration form covers the holidays.
Questions
How do families with two children register?
They send the form once per child. Each response is one row, so siblings share guardian details across two rows. Sort the CSV by guardian_email to match brothers and sisters.
Can the form check a birth certificate or proof of address?
No. It cannot take uploads or check documents. Ask families to bring papers to the office, and use the form for details you copy into your student system.
Can I stop registrations when classes are full?
Yes. Set max_responses to your intake size and the form stops at that count. Use close_at as well if registration also has a date.
