A new class is a seating plan and a list of names. A student profile adds the person behind each name. I send one link, students answer in their own words, and I read the sheet before the next lesson.
How it works
- The student gives the name they like to be called, and their class.
- They pick favourite subjects, how they like to learn and what they enjoy outside school.
- They share something they are proud of and a goal for the year.
- A parent or guardian adds a contact and how they like to be reached.
A short sample chat
A student's AI might ask: "What name should your teacher use?" Then "Which three subjects do you like most?" Then "What is one goal for this year?" It shows the answers and sends only after a yes. See agent-fillable forms.
Good to know
Keep learning styles light. Students say what feels comfortable, not what they can or cannot do. Read the pride and goal answers first and use them in your first chat with each child. Keep office facts in a student registration form, and save this one for personality. Ask for views mid-year with a student feedback form, and offer clubs through a student club sign-up. More are in forms for schools.
Questions
Should children fill this in alone?
Younger children may need a grown-up to read the questions aloud. Older students can answer on a phone. The parent contact boxes are for the adult to complete.
How do I use the answers?
Filter the CSV by favourite subjects or interests to form groups, choose project topics or pair partners. Read the goal column before parent meetings.
Is this the same as a registration form?
No. Registration collects what the office needs. A profile collects what a teacher wants to know: interests, goals and how a child likes to learn. Use both if you need both.
