The first lesson goes better when I already know the level, the goal and the trouble spots. So I send this form first. It works as a tutoring intake form too. The family answers at home, and I arrive prepared.
Who it is for
Private tutors, small learning centres and teachers who take on new students. It suits one subject or several.
What each question is for
- Year group and subject: so you bring the right materials.
- Confidence rating and recent result: a quick view of how the learner feels and where they stand.
- Goals and exam date: catching up and exam prep need different plans.
- Hard topics and what helps: the gaps, and the way this learner likes to work.
- Earlier tutoring: tells you what has already been tried.
What a family's AI would ask
"Which year group is the learner in?" Then "How confident do they feel in the subject, from 1 to 5?" Then "When is the next exam?" It shows all the answers and sends only after a yes. Read about agent-fillable forms.
Before you use it
This records what the family says. It is not a test, so run your own short check in lesson one. Once lessons start, share results with a progress report and collect opinions with a student feedback form. More in forms for tutors and childcare.
Questions
Is this a test of the student?
No. It records what the family tells you: confidence, recent results and hard topics. Run your own short check in the first lesson to see where the learner really is.
How do I plan the first lesson from the answers?
Download the CSV and read hardest_topics and tutoring_goals first. The exam_date column shows who needs a plan with a deadline, so start with those learners.
Can a parent fill it for an older student?
Yes. Whoever fills it can say so in the notes. A parent can also give the form to their own AI agent, which asks for each detail and shows all answers first.
