New hire checklist form: first-week setup in one link

The first week is full of small things that nobody checks. The laptop works, but the building pass does not. I like a short checklist that the new starter fills in on day three or four. It shows me which setup step failed while it is still easy to fix.

Who it is for

Managers, office managers and small HR teams who onboard people without a big system. Also IT, who need to know which accounts are still missing.

What each question is for

Setup steps done: a tick list that shows which step slipped.
Equipment and tools: the gaps IT can close this week.
Buddy: whether the new person has someone to ask.
Clarity rating: one number to compare across starters.
Blocked box: puts urgent cases at the top of your list.
Day one idea: what to change before the next hire.

What a new hire's AI would ask

Before you use it

Send the link on day three, not day one. When someone leaves, an offboarding checklist is the matching exit form. Staff who drive for work may also need a mileage log. More ideas are in HR and recruiting forms and IT and operations forms. This form does not store contracts or identity papers.

When should I send the checklist?

Send it on day three or four, when most gaps have shown up. Set `close_at` to the end of the first week so you work from answers of the same age.

Can a manager fill it in for the new hire?

Yes. Anyone with the link can answer, so a manager can fill it in during a one to one. The new hire's own AI can also fill it and show the answers first.

How do I spot problems across several starters?

Download the CSV and read the `clarity_rating` and `still_blocked` columns. Low scores and ticked blocks point to the setup steps worth fixing for the next hire.
