A message that says "I want guitar lessons" tells a teacher almost nothing. Acoustic or electric? Beginner or rusty? Which songs? This template asks those things up front, so the first reply can be useful instead of a list of questions.
How it works
- The student gives their name, age and email, plus a parent or guardian name if they are under 18.
- They pick an instrument and say how much they have played.
- They say whether they own an instrument, which styles they like and what they want to play first.
- They choose a lesson length, free days and a first lesson date.
A short chat
A student's AI might ask: "Which instrument do you want to learn?" Then "Have you played before?" Then "What is the first song you want to play?" It shows the answers and asks "Send it?" before anything goes out. See agent-fillable forms.
Good to know
This works for any instrument, not only guitar. Edit the options to match what you teach. Sort the CSV by the instrument column, and answer students who do not own one with buying advice first. The form takes no payment, so agree fees your usual way. Offer group classes too? Try a workshop registration form, or the general tutoring sign-up form for other subjects. More in forms for tutors and childcare.
Questions
Can I use it for piano or drums?
Yes. The instrument list holds guitars, keys, drums, voice and violin. Edit the options to match what you teach before you share the link.
Why ask whether the student owns an instrument?
It shapes the first lesson. A student without a guitar needs advice on borrowing or buying before any chords. Filter the owns_instrument column to find them.
How do I handle students under 18?
The form asks for a parent or guardian name next to the age. Keep everything else to the minimum, and agree the lesson details with the parent.
