Memory is a poor witness to a Tuesday lunch. I would rather note each meal while it is still on the plate. The entries then sit in one list that a coach or a dietitian can read.
Before the first entry
Decide who reads it. It may be only you, a coach or a dietitian. Tell people to log as they go, because food forgotten at lunch is hard to find at night. Save the link to the phone home screen.
While you eat
Each entry stays short. Date, which meal and roughly when. Then what you ate and drank, in your own words, and the portion. Hunger before and fullness after show the pattern behind the plate. Mood, place and any feelings afterwards add context. A water count is optional.
Talking is quicker for some people. Your AI might ask "What did you have for lunch?" Then "How hungry were you before?" Then "Did you feel bloated afterwards?" It shows the whole entry and sends it only after a yes. Read about agent-fillable forms.
After a week
Download the CSV. Read the notes, sort by meal, and look for repeats. Take the file to the person who helps you. Check your local privacy rules before collecting health data. This is a form, not medical advice or nutrition advice.
Fitness teams can pair it with a workout log and a fitness assessment form. See forms for gyms and forms for clinics.
Questions
Does the form count calories?
No. It records what you ate in your own words, plus context like hunger and mood. A dietitian or coach can read the pattern. It gives no nutrition advice.
How can a coach read a client's diary?
The client downloads the CSV and shares it, or the coach owns the form and each client uses the link with a name added to their notes. Agree on privacy first.
How do I find which meals cause trouble?
Download the CSV and filter felt_after for anything other than Nothing unusual. Then read food_and_drink on those rows and look for repeats.
