Paper slips are easy to lose, and I want to read the answers before tomorrow. A link on the board does the job. Students answer on a phone, or hand the form to their own AI.
How it works
- Students name the main idea in their own words.
- They rate how well they follow it, from lost to could teach it.
- They say what is still unclear and how the pace felt.
- They leave one question for tomorrow.
A short sample chat
A student's AI might ask: "What was the main idea today?" Then "How well do you follow it, from 1 to 4?" Then "What is still unclear?" It shows the answers and sends only after a yes. See agent-fillable forms.
Good to know
Sort the CSV by the rating and open tomorrow's lesson with the lowest rows. Pace and "what helped" tell you which activities to repeat. Browse forms for schools and forms for tutors. Once a term, ask for a longer view with a student feedback form, and share progress at home with a progress report.
Questions
How do I find the students who struggled today?
Download the CSV and sort the understanding_level column from low to high. The lowest rows are the students to check first. Read still_unclear beside each score for the reason.
Should students put their names on it?
Name is optional in this template. Unnamed tickets tend to be honest, while named ones let you follow up. Make the name field required if you plan to reply to each student.
Can one form cover a whole week of lessons?
Yes. Students type the lesson topic each day, so that column separates the days. For a clean sheet per lesson, create a new form each time.
