The award often goes to whoever is loudest or most visible. I would rather ask for a real example and the difference it made. This employee of the month template collects both from any colleague, then hands you a list to decide from.
Use this when
- You want colleagues, not only managers, to say who stood out.
- You need nominations inside a deadline each month.
- You want the story behind each name, to read out at the announcement.
Skip it when
You just want a quick thank-you in a chat channel. A form is more than that needs. Skip it too if nobody will read the stories.
What is asked, and why
- Nominee and team: so the right person is counted.
- How the nominator knows them: it helps you weigh the view.
- Values shown: it ties each nomination to what you care about.
- One example and its impact: this separates a nice colleague from a real difference.
- Permission to share: you only read out words that the writer allowed.
What a nominator's AI would ask
"Who do you want to nominate?" Then "Which of our values did they show?" Then "What did they do? Give one example." It lists everything and waits for a yes before it sends. See agent-fillable forms.
Next steps
Set the close date at month end. Welcome new people with an employee onboarding form, and let staff bring in friends through an employee referral form. More in forms for HR and recruiting.
Questions
Can people nominate themselves?
You decide. The form does not stop it. If you do not want self-nominations, say so in the form description and compare the two name columns in the CSV.
How do I choose a winner fairly?
Download the CSV, count nominations per nominee, then read the examples and impact scores. A clear written example should count for more than a vote.
Do nominations stay private until the announcement?
Yes. Only you, as the form owner, read the responses. Set a close date at the nomination deadline so the list is final when you decide.
