AI forms for photographers

Good sessions start with good questions: what is the day, who is there, which shots matter. These templates collect that before you pick up the camera. They are forms, not legal advice.

What these forms do for photographers

• Booking and questionnaire forms replace long email threads.
• Release forms record a typed name and a checkbox.
• Selection and feedback forms keep client choices in one list.

Start with these

• Photo release form: A photo release form for shoots: who gives permission, where the photos may be used, how to credit them and an agreement with a typed name. Export as CSV.
• Photo contest entry form: A contest entry form for photo contests: entrant details, category, photo link, where and when it was taken, editing level and rule ticks. Export as CSV.
• Portrait session questionnaire: A photography questionnaire for family, senior and newborn portrait sessions: who is in the photos, style, outfits, must-have shots and practical needs.
• Photography booking form: A photography booking form for enquiries: type of shoot, preferred and backup date, location, length, people, photo use and a delivery deadline. Export as CSV.
• Wedding photography questionnaire template: A wedding photography questionnaire: venues, timings, coverage, shot list, family groups, style and family notes for the photographer. Export as CSV.
• Photographer application form: A photographer application form for second shooters and assistants: portfolio link, experience, styles, gear, free dates, travel range and why they want to join.

One tip

Send the questionnaire when you confirm the date, and read the answers the evening before the shoot.

Your AI or theirs

Your AI agent can create any of these forms and hand you the link. The people who fill it can use their own AI too. Read how that works in agent-fillable forms. Looking for a different trade? See all industries or browse every template.
