A new client at the front desk holds a restless pet and a half remembered vaccine date. I would rather the owner answer at home, with the vaccine card in hand.
Who it is for
Small vet clinics, mobile vets and animal hospitals that want the basics before the first appointment. It works as a vet intake form for dogs, cats and other pets.
What each question is for
- Owner name, phone, email: to reach the right person about the right pet.
- Pet name, kind, breed, birth date and sex: to open a record.
- Reason for the visit: tells the nurse what to prepare.
- Vaccines, medicines, allergies and diet: the facts owners forget in the room.
- Previous clinic and records box: lets you ask for old records.
- Worries: lets the owner say what matters most.
What an owner's AI would ask
"What is your pet's name and breed?" Then "Are vaccinations up to date?" Then "Why are you coming in?" It shows the answers and sends them only after a yes. See agent-fillable forms.
Before you use it
Check your local privacy rules before collecting health data. This is a form, not medical advice. Browse forms for pet businesses and rescues and forms for clinics and healthcare. Pair it with the general client intake template for other details, and a pet sitter form if you board animals.
Questions
Does this replace my practice software?
No. It collects answers and gives you rows as JSON or CSV. You copy what you need into your own records, where clinical notes belong.
Can an owner skip the vaccine date?
Yes. Vaccination status is a choice, and the exact dates stay off the form. Ask for the vaccine card at the visit, or request records from the previous clinic.
How can I prepare for visits from the CSV?
Sort by the first_visit_reason column. Group wellness visits and vaccinations, and read the allergies and medicines columns before anyone arrives.
