Questions

Does the form ask for the learner's name?

No. We store a salted hash of the visitor IP address, so do not promise full anonymity.

Can I use one form for many courses?

Yes. The course_name field tells them apart. Filter the CSV by it.

How do I compare instructors?

Download the CSV. Average the three rating columns for each instructor_name.
